We added detailed description of the event, to ensure that clues to the reason for the fall is detected. Important aspects would be dizziness (possibly indicating arrhythmias, hypotension, disorders of balance, or syncope), loss of consciousness (indicating a cerebral problem), palpitations (raising the possibility of arrhythmias), incontinence (possibly suggesting seizures), weakness (perhaps indicating a musculoskeletal or neurological problem), and tripping.

It would also be important to ask about alcohol and illicit drug use, and use of prescribed medication.

If necessary add to your Assessment Sheet as you will be able to save this out to act as an aide memoir.
